Planned Absence (Family Vacation) Request for Assignments

Planned Absence Form

Use this form if you are planning on keeping your child out of school for an extended period, such as a planned family vactation.

Enrollment Forms

Enrollment for NEW "ELEMENTARY" STUDENTS at Our Savior's Lutheran School begins with filling out and turning in an "Elementary Admission Application ." If accepted, you will receive an Enrollment Package containing additional forms to be filled out and returned.
